Between October 2000 and May 2002, from a well-defined 25 km2 geographical area, we recruited 11,746 men and women (original cohort) who were 1) married (to reduce loss to follow-up) and aged 18–75 years, 2) residents of the study area for at least 5 years prior to recruitment, and 3) primarily drinking water from a local well.
